A) Rockridge Building LLC on or about January 24, 2018, at the address of 9616 Stallion Circle, Whitehouse, Ohio: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards, such as falls during job site operations, decreasing the probability of employee injury and/or illness.

A) Rockridge Building LLC on or about January 24, 2018, at the address of 9616 Stallion Drive, Whitehouse, Ohio: The employer did not ensure employees were protected from head injuries while walking/working below employees operating from moving platform above. Employees were exposed to potential impact injuries caused from falling material and/or equipment.    B) Rockridge Building LLC on or about January 24, 2018, at the address of 9616 Stallion Drive, Whitehouse, Ohio: The employer did not ensure an employee was protected from a head injury, while working at a level horizontal plain, when an aerial lift was in motion. The employee was exposed to potential impact injury caused from moving heavy equipment (fork lift).

A) Rockridge Building LLC on or about January 24, 2018 conducting work at 9616 Stallion Drive, Whitehouse, Ohio: The employer did not ensure a platform / pallet was securely attached to the forks.  The unsecured platform was at a height of 15ft when the employees were applying sheeting (OSB) to the roof trusses.  B) Rockridge Building LLC on or about January 24, 2018 conducting work at 9616 Stallion Drive, Whitehouse, Ohio: The employer did not ensure a platform / pallet was free of horizontal movement, while occupied by two employees. The unsecured, moving platform was at a height of 15ft when the employees were applying sheeting (OSB) to the roof trusses.

The employee was exposed to a potential 15 foot fall due to the weak working position.    B) Rockridge Building LLC on or about January 24, 2018 at the address of 9616 Stallion Drive, Whitehouse, Ohio: The employer failed to ensure an employee did not sit on the front edge of the pallet platform while bracing against the roof trusses creating an unsafe working position. The employee was exposed to a potential 15 foot fall due to the weak working position.

A) Rockridge Building LLC on or about January 24, 2018 at the address of 9616 Stallion Drive, Whitehouse, Ohio: The employer did not ensure the side rails of the A-frame ladder (collapsed A-frame 6 foot ladder) was extended at least three feet above the landing surface. One employee was exposed to falls while using the ladder to access the top plate in route to the moving scaffold.

Rockbridge Building LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1211298, citation number 1, item number 1 and was affirmed as a final order on May 18, 2017, with respect to a workplace located at 727 Prairie Rose, Monclova, Ohio, 43542.
