29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:    a) 827 Yellowstone River Rd: On January 24, 2018 and at times prior, employees were exposed to catapult and subsequent fall hazards of approximately 17 feet while installing metal roofing from the 400S JLG Aerial Lift.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:  a) 827 Yellowstone River Rd: On January 24, 2018 and at times prior, employees were exposed to fall hazards of approximately 17 feet 9 inches while installing metal roofing.
