29 CFR 1910.219(d)(1):  Pulleys, any parts of which were seven (7) feet or less from the floor or working platform, were not guarded in accordance with the standards specified in paragraphs (m) and (o) of this section:  At 13260 Corporate Exchange Drive, St. Louis, MO, specifically at the Unloading Area, the belt and pulley belonging to the Brock Super-Air Pneumatic Systems, m/n 0-48877-3, s/n 31009729, was not adequately guarded in that the guard was secured into place with butterfly nuts.   These butterfly nuts and guard were removed by hand by an employee on or about 01/24/18 for the purpose of clearing material that jammed the auger belonging to this system.  The employee manipulated the belt with his hands that resulted in an amputation of the tip of his right thumb. This violation was noted on 01/30/18.
