29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements of subpart I of this part were not worn or attached to the boom or basket when working from an aerial lift.   On the west side of the parking lot, an employee working from a Genie man-lift without fall protection was exposed to falling more than 21 feet to the lower level.

29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects.    On the west side of the parking lot, an employee not wearing a hardhat while working below another employee on Genie man-lift performing painting activities was exposed to struck-by hazards from falling objects.
