29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): a) CLDI Project, South Wall: On or about January 23, 2017 and at times prior, employees were exposed to struck by hazards while drilling anchor bolt holes below an unsupported concrete beam. The employer failed to have a competent person conduct adequate inspections of the job site to identify potential structural collapses prior to the start of the work, and as needed during the work process to address unforeseen conditions.

The employer failed to provide adequate training in the recognition of demolition hazards and the means to control these hazards.
