29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a. On or before January 25, 2018, where employees operated the cutoff saw, the employees were exposed to the blade of the saw, thereby being exposed to a caught-in or other hazards.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets. a. On or before January 25, 2018, where employees were worked on or near the unscrambler for the double head notcher, the tensioning sprocket for the chain drive was not guarded, thereby exposing employees to caught-in hazards. b. On or before January 25, 2018, where employees were worked on or near the cut-off saw, the sprocket and chain for the conveyor coming from outside was not guarded, thereby exposing employees to caught-in hazards.

29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). a. On or before January 25, 2018, the employer failed to ensure that employees that used powered industrial trucks to move lumber and pallets around the facility were trained in the safe operation of the powered industrial truck, thereby exposing employees to struck-by hazards. Valley View Pallets, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.178(l)(1)(ii) which was contained in OSHA inspection number 1120523, citation number 1, item number 3 and was affirmed as a final order on March 16, 2016, with respect to a workplace located at 22414 Hostetler Road, Danville, OH 43014.
