On or about January 25, 2018, at the site located at building 11 Lakes at Cheshire Drive, Delaware, Ohio, an employee was loading lumber onto the second story of the apartment building through an unprotected wall opening approximately 10 feet in height with no fall protection, thereby exposing the employee to a fall hazard.
