29 CFR 1910.28(b)(1)(i): (1) Unprotected sides and edges.  (i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:      Employees working inside the Transfer Station Building near the edge of the loading pit were exposed up to a 8 ft.  fall hazard without the use of any fall protection system.

29 CFR 1910.30(a)(1): (a) Fall hazards. (1) Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart.  Employers must ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.      The employer did not ensured that employees working in the Transfer Station building were trained on the fall hazard while working near the edge of the loading pit.

29 CFR 1910.178(n)(10): The driver shall be required to slow down for wet and slippery floors.    On January 19, 2018, inside the Transfer Station Building, a Hyster sit-down forklift operator was travelling on the wet, sludge, and trash laden concrete floor inside the transfer station and was attempting to make a right turn to the dock area to unload a tractor trailer of pallets when the forklift skidded in a straight line into the trailer pit.
