29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolding was not fully planked or decked:    On or about January 24, 2018, at the worksite located at 11 Prospect Court in Cabot, Arkansas, on the supported scaffold located on the southwest side of the building, employees worked from two abutted wooden boards supported atop a building wall cornice and scaffold platform to perform EIFS foam cornice board installation tasks in lieu of a fully decked and planked platform.  This condition exposed employees to falls from elevation.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:    On or about January 24, 2018, at the worksite located at 11 Prospect Court in Cabot, Arkansas, on the supported scaffold located on the southwest side of the building, employee accessed and dismounted the 3rd level platform by climbing the scaffold end frames.  This condition exposed employees to slips and falls from elevation.

29 CFR 1926.451(g)(2):  The employer did not provide fall protection for employees erecting or dismantling supported scaffolds:    On or about January 24, 2018, at the worksite located at 11 Prospect Court in Cabot, Arkansas, the designated competent competent person did not conduct a feasibility assessment to ascertain the need for fall protection to be provided to employees engaged in scaffolding erection activities.  Employees on the 3rd level platform of the supported scaffold located on the southwest side of the building performed scaffolding erection tasks without the use of fall protection while exposed to unprotected falls of greater than 10 feet.

29 CFR 1926.451(g)(4): Guardrail systems installed to meet the requirements of this section shall comply with the following provisions (guardrail systems built in accordance with Appendix A to this subpart will be deemed to meet the requirements of paragraphs (g)(4)(vii), (viii), and (ix) of this section):  On or about January 24, 2018, at the worksite located at 11 Prospect Court in Cabot, Arkansas, on the supported scaffold located on the southwest side of the building, the guardrail system for the approximately 2 1/2 feet open area on the 3rd level platform was not equipped with a midrail to protect employees traveling in the area carrying a guardrail component and engaged in EIFS plastering and foam cornice board installation tasks.  Employees were exposed to falls from elevation.
