29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    Northwest, tray/slice room, Marchant Schmidt, Inc. tray line conveyors and ultrasonic cutters - On or about January 12, 2018, employees assigned to perform servicing and maintenance activities on energized equipment were not provided with adequately developed and documented machine-specific hazardous energy control (lockout tagout) procedures.  The procedures lacked specific steps to isolate and control hazardous energy sources, steps for the placement of lockout devices, and specific verifications steps to determine the effectiveness of the energy control measures.  Employees engaged in activities such as, but not limited to, cleaning tasks were exposed to machine hazards likely to result in serious injuries due to in-going nip points and shear points.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    (1) prepare for shut down by recognizing energy hazards and its controls,  (2) turn off and shut down the machines,  (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,  (4) affix lockout or tagout devices to each energy isolating device,  (5) render safe all potentially hazardous stored or residual energy,  (6) verify that isolation and de-energization of the machine or equipment have been accomplished. 29 CFR 1910.332(b)(1):  Employees shall be trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertain to their respective job assignments:     Entire Facility - The employer did not ensure that employees were trained in and familiar with safety-related work practices, such as the use of special precautionary techniques, personal protective equipment (PPE), insulated tools and insulating and shielding materials necessary to safely accomplish such tasks. Employees were exposed to the hazards of electrocution, electrical shock and arc flash/blast injuries. Maintenance employees performed troubleshooting and servicing tasks on electrical components, including changing blown fuses and voltage testing on energized components up to 480V. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    Northwest, tray/slice room, Marchant Schmidt, Inc. tray line conveyors:  (a) tray line conveyor slicer 4, model M03109001, serial no. 35028 026, connecting to tray turn checkweigh conveyor M03063001, serial no. 35028 012;  (b) tray line conveyor slicer 3, model M03108001, serial no. 35028 025;  (c) tray line conveyor slicer 2, model M03107001, serial no. 35028 024;  (d) tray line conveyor slicer 1, model M03106001, serial no. 35028 023;     Employees were exposed to in-going nip points and caught-in hazards from unguarded horizontal flat belts and sprocketed drive shafts on the above referenced Marchant Schmidt, Inc. tray line conveyors.  Interlocked barrier guards were removed or otherwise bypassed for production purposes, exposing employees to serious injury in the event they inadvertently contacted the danger zone associated with the operating tray line conveyor belts.
