29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. On January 10, 2018 and prior to that time employees were performing structural steel erection, structural steel sling rigging and structural steel material handling and storage without being trained on the hazards associated with the operations or applicable regulations.

29 CFR 1926.754(a): Structural stability shall be maintained at all times during the erection process. On November 13, 2017 during the initial erection of structural steel columns, open web joist girders and open web tie joists along B line, open web joist girders were connected to the columns from 1 to 4 lines prior to any open web tie joist being connected to the tilt wall panels at A line at which time structural stability of the columns, open web joist girders and open web tie joist was not maintained subsequently causing the head weight of the open web girders not to be adequately supported by the columns within 6 inch column sleeves, resulting in the erected columns and connected open web joist girders collapsing to the inside of the building at C line exposing employees to a struck by hazard potential of collapsing structural steel.

29 CFR 1926.757(a)(6): When steel joist(s) are landed on a structure, they shall be secured to prevent unintentional displacement prior to installation. On January 10, 2018 located at column line F42 between 41 and 42 an employee performing a column rigging operation was working between the column and an unsupported open web joist girder which had been previously vertically landed on the structure without being secured from unintentional displacement prior to installation exposing the employee to a struck by caught between hazard created by the open web joist girder being displaced and falling over.
