29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder. A). On or about January 26, 2018 two employees had set a 24 foot aluminum, Werner brand Extension ladder, on the guard shed at the entry to a new residential construction site and had not secured the ladder to the structure to prevent accidental displacement.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a. The employer fails to assure effective protection against falls as required by the standard, as demonstrated on January 26, 2018, when two employees are working on a 4:12 steep slope, commercial roof at the entrance to a new residential construction development, were not protected from a 10'-3" fall hazard by the use of guard rails, safety nets or personal fall protection system. Stuart Lyons Roofing, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10) which was contained in OSHA inspection number 952096, citation number 1, item number 1, and was affirmed as a final order on January 10, 2014, with respect to a workplace located at 201 Grenada Blvd, Ormond Beach, FL 32174.
