On or about January 18, 2018, and times prior, the Safe Harbors TLS Gangway, used to gain access to and from railcars, had unprotected sides of 31 inches in width, exposing employees to falls of 12 feet 6 inches to ground.

On or about January 25, 2018, and times prior, the safety cage attached to the TLS Gangway provided a top rail but did not provide a mid-rail for the up to 30in. by 31in. wide opening on both sides, exposing employees to falls up to 12ft. 6in. to the ground below.
