29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a) On or about and at times prior to January 19, 2014, a damaged flexible cord was used to connect a concrete/cement mixer to a temporary power outlet, exposing the employees who operate the mixer to an electrical shock hazard.
