(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:     On or about January 22, 2018 and at times prior, at the racking board platform of Rig 219, the emergency descent device available for egress from the derrick would not extend due to a mechanical deficiency preventing its use as a means of escape.

29 CFR 1910.307(c)(2)(i): Equipment in hazardous (classified) location(s) was not approved for the ignitable or combustible properties of the specific gas, vapor, dust, or fiber that was or could be present:  On or about January 21, 2018, at the rig floor, within 10 feet of the well bore, a heat lamp used at the iron Roughneck was not approved for use in a clsss 1 div 2 hazardous location.  Employees  worked at the rig floor during gas well drilling operations and were exposed to fire and explosion hazards.

1910.1030(f)(1) The employer did not have procedures  for  all employees who have occupational exposure, and post-exposure evaluation and follow-up to all employees who have had an exposure incident.    On or about January 22, 2018, at the welsite, the employer's Bloodborne Pathogens Exposure Control Plan did not include procedures for post-exposure evaluation and follow-up for a exposure incident for blood or other potentially infectious materials.  Employees were required to provide first aid and had the potential for occupational exposure to blood or other potentially infectious materials.
