29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.:  (a) On or about 05/24/2018, the employer failed to submit documentation and certification of corrective action for citation 1, item 1 issued on 02/27/2018 with an abatement due date of 03/05/2018.  (b) On or about 05/24/2018, the employer failed to submit documentation and certification of corrective action for citation 1, item 2 issued on 02/27/2018 with an abatement due date of 03/05/2018.  (c) On or about 05/24/2018, the employer failed to submit documentation and certification of corrective action for citation 1, item 3 issued on 02/27/2018 with an abatement due date of 03/05/2018.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  (a) On or about 01/26/2018, 817 South Bend Ln, Vestavia Hills, AL 35216, employees were exposed to fall hazards above the ground of approximately 20 feet at the third level and 13 feet 5 inches at the second level while performing bricking operations from a fabricated frame scaffold system that was not fully planked.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:   (a) On or about 01/26/2018, 817 South Bend Ln, Vestavia Hills, AL  35216, employees were exposed to fall hazards while climbing end frames of the scaffolding system approximately 13 feet, 5 inches above the ground.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a) On or about 01/26/2018, 817 South Bend Ln, Vestavia Hills, AL 35216, an employee was not protected from falling approximately 20 feet while catching bricks being tossed to him on the third level of a fabricated frame scaffold system without fall protection.
