29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) For employees working at building 404 as observed on or about 25 January 2018. Employees were observed on the roof without the use of a fall protection system in use. Employee ascended ladder and stepped onto roof to deliver a piece of lumber to employee already on roof. The employee was working on an approximate 5:12 pitch roof and exposed to a fall height of approximately 18 feet 3 inches.     b) For employees working at building 404 as observed on or about 25 January 2018. Employees were observed on the roof without the use of a fall protection system in use. Employee was working on the roof while connected to a lifeline. The employee disconnected the lanyard from the harness and descended down the roof to the ladder. The employee was working on an approximate 5:12 pitch roof and exposed to a fall height of approximately 18 feet 3 inches.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.  b) For employees working at building 404 as observed on or about 25 January 2018. Employees were observed on the roof with the use of a fall protection system in use. The employer did not know the strength of the knot or the rope in use.The anchor for the lifeline in use was wrapped around a tree and knotted onto itself.  The employee was working on an approximate 5:12 pitch roof and exposed to a fall height of approximately 18 feet 3 inches.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) For employees working at building 404 as observed on or about January 25, 2018. Employees were observed using one extension ladder to access the upper working roof surface. The ladder side rails were extended approximately 2 feet above the upper landing surface. The employees were working on an approximate 5:12 pitch roof and exposed to a fall height of approximately 18 feet 3 inches.
