29 CFR 1926.251(a)(4): Special custom design lifting accessories for material handling were not marked to indicate the safe working loads and were not proof tested prior to use to 125 percent of their rated loads:     (a) Worksite:  On or about 1/29/2018, a Link-Built crane, used to lift a 1.5 ton wrecking ball in a demolition project, was connected to the wrecking ball by using two truck tires with cables wrapped around the top and bottom of the tires. The tires were not proof tested prior to being put into use, nor were they marked to indicate their safe working capacity.

29 CFR 1926.251(c)(5): U-bolt wire rope clips used to form eyes did not conform with Table H-20 to determine number and spacing of clips:   (a) Worksite:  On or about 1/29/2018, the employer did not ensure that the crane rigging cable supporting the weight of a 1.5 ton wrecking ball used the appropriate number of U-bolts, in accordance with Table H-20, to form eyes.
