Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards.    On or about January 22, 2018, and times prior thereto, on Patterson-UTI Rig #219 near Quinton, Oklahoma where well control was not maintained during drilling operations at the Pryor Trust 0718 Well Number 1H-9.

29 CFR 1910.120(q)(1): Emergency response plan. An emergency response plan was not implemented to handle anticipated emergencies prior to the commencement of emergency response operations.  On or about January 22, 2018 and at times prior, at the well site, an emergency response plan was not implemented for uncontrolled releases of hazardous chemicals including, but not limited to natural gas and diesel containing drilling mud during gas well drilling operations which exposes employees to fire and explosion hazards.

29 CFR 1910.165(c)(2): Spare alarm devices and components subject to wear or destruction were not available in sufficient quantities and locations for prompt restoration of employee alarm systems:  On or about January 22, 2018 and at times prior, at the well site, a back-up employee alarm system was not provided where more than 10 employees were present.  No employee alarm for emergency evacuation was activated after gas well blowouts which expose employees to fire and explosion hazards.

29 CFR 1910.307(c)(2)(i): Equipment in hazardous (classified) location(s) was not approved for the ignitable or combustible properties of the specific gas, vapor, dust, or fiber that was or could be present:  On or about January 21, 2018 and at times prior, at the rig floor, within 10 feet of the well bore, a heat lamp used at the Iron Roughneck was not approved for use in a class 1 div 2 hazardous location.  Employees worked at the rig floor during gas well drilling operations and were exposed to fire and explosion hazards.
