On or about 1/29/2018 a) Construction material such as, but not limited to, concrete blocks, masonry cement and other construction materials were stored on the 3rd floor and the maximum safe load was exceeded and a collapse occurred. Employee were injured.

Location: 404 11th Street, Brooklyn, NY; 3rd floor. On or about 1/29/2018 a) The employer did not determine if the walking/working surface was capable of supporting the employees and materials stored on the floor area. An overloading of the floor caused the floor to collapse and an employee was partially buried and injured.

Location: 404 11th Street, Brooklyn, NY; 3rd floor. On or about 1/29/2018, a) An employee attached his safety harness to the framing studs of an interior wall when working next to a wall opening. Employee exposed to a fall of approximately 24 feet.
