Location: 404 11th Street, Brooklyn, NY.    On or about 1/29/2018    a) Prior to storing construction materials on the work floor, an inspection of the worksite was not done to determine if the maximum capacity of the floor will be exceeded by the additional load.      b) Framing studs were used as an anchorage for a personal fall arrest system

29 CFR 1926.250(a)(2): Maximum safe floor load limits were exceeded.    Location: 404 11th Street, Brooklyn, NY.    On or about 1/29/2018    a) Construction material such as, but not limited to, concrete blocks, masonry cement and other construction materials were stored on the 3rd floor and the maximum safe load was exceeded and a collapse occurred.  Employee were injured.

29 CFR 1926.501(a)(2): The employer did not determine if the walking/working surfaces on which its employees were to work had the strength and structural integrity to support employees safely.    Location: 404 11th Street, Brooklyn, NY; 3rd floor.    On or about 1/29/2018    a) The employer did not determine if the walking/working surface was capable of supporting the employees and materials stored on the floor area.  An overloading of the floor caused the floor to collapse and an employee was partially buried and injured.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.    Location: 404 11th Street, Brooklyn, NY; 3rd floor.    On or about 1/29/2018,    a) An employee attached his safety harness to the framing studs of an interior wall when working next to a wall opening.  Employee  exposed to a fall of approximately 24 feet.
