OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to the hazard of fire: On or about January 23, 2018 at the cotton gin employees engaged in lint cleaning operations were exposed to fire hazards.

29 CFR 1928.57(a)(11): Where a moving machinery part presented a hazard during servicing or maintenance, the engine was not stopped, the power source was not disconnected, and all machine movement was not stopped before servicing or maintenance was performed, except where the Employer could establish the following in 1928.57(a)(11)(i-iii): On or about January 23, 2018 at lint cleaner #1B employees engaged in maintenance were exposed to caught-in hazards because lint cleaner was not locked-out/tagged-out.
