29 CFR 1926.960(c)(1)(iii): The employer did not ensure that no employee approaches or takes any conductive object closer to exposed energized parts than the employer's established minimum approach distance, unless the measures outlined in 1926.960(c)(1)(iii)(A) - (C) were met: 1) Southwest side of Phoenix Boulevard and Phoenix Parkway: On or about January 25, 2018 and at times prior, the employer exposed employees to burn hazards, in that employees were installing wires inside a switch gear box with energized parts over 7200 volts without insulating the equipment. The employees sustained 1st, 2nd and 3rd degree burns.

29 CFR 1926.960(g)(1): The employer did not assess the workplace to identify employees exposed to hazards from flames or from electric arcs. 1) Southwest side of Phoenix Boulevard and Phoenix Parkway: On or about January 25, 2018, and at times prior, the employer exposed employees to burn hazards, in that employees were pulling wires inside a switch gear box S4 model 55352R3-K2 and model 9722270, where the top fuses were energized, and an assessment had not been conducted. The employees sustained serious injuries from an arch flash.
