29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    a) Honda of America Mfg., Inc. on or about January 24, 2018, at the address of 12500 Meranda Rd. Anna, OH: The employer did not ensure that energy isolating procedures were implemented prior to performing service and maintenance activities inside the Line #1 Robotic Cell. A maintenance employee performing these tasks in a condition where a lockout device was not placed on the cells main electrical disconnect was exposed to the operating radius of the robotic arm. The employers established energy control procedure [required under 29 CFR 1910.147(d)] was not adequately utilized before the employee began the aforementioned work task inside the robotic cell.

29 CFR 1910.147(f)(1): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions in 29 CFR 1910.147(f)(1)(i-v) were not followed:   a)  Honda of America Mfg., Inc. on or about January 24, 2018, at the address of 12500 Meranda Rd. Anna, OH: The employer did not ensure that employees were protected from energized components of the FMD Spin Line #1  Robotic Cell during testing activities. An employee performing calibration of the pneumatic metering cylinders contained in cell #1, was exposed to amputation hazards in that the employees hand was able to be in proximity of a pinch point near the point of operation. During this testing task, the employer did not ensure the employee was removed from the hazard or alternatively protected by means of effective machine guarding.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:    a) Honda of America Mfg., Inc. on or about January 24, 2018, at the address of 12500 Meranda Rd. Anna, OH: The employer did not ensure that maintenance (authorized) employees tasked with service and maintenance activities at or near energized parts inside the FMD Spin Line #1  Robotic Cell adequately understood the application of lockout procedures in which isolate and control hazardous energy sources during both maintenance and testing tasks. Employees not understanding energy isolation application and testing procedures were exposed to energized components of the cell.
