29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: On or about January 09, 2018 and times prior thereto, at the site located at or near 2800 Great Dane Drive in Jonesboro, Arkansas, the front guards on the Piranha Aluminum Panel Sheer in the Assembly 3 area were bent allowing the operator's hands to enter the danger zone during the operating cycle. This condition exposed employees to amputation hazards.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. On January 09, 2018 at the worksite located at or near 2800 Great Dane Drive in Jonesboro, Arkansas, an employee received an injury resulting in an amputation while operating the aluminum panel shear. The employer failed to report the inpatient hospitalization to OSHA within twenty-four (24) hours as required.
