29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:   a) Facility; on or about January 31, 2018 and times prior, the employer failed to develop and implement a hazard communication program to protect employees from hazardous materials such as, but not limited to Clean Strip J1A and Shell Rotella T Triple Protection 10W-30 oil.   Murrill Tank Service, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard.  1910.1200(e)(1), which was contained in OSHA inspection number 966605, citation number 1, item number 1 and was affirmed as a final order on May 9, 2014, with respect to a workplace located at 2850 Hamilton Blvd, Mobile, AL 36605.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification that identified the workplace evaluated; the person which certified that the evaluation had been performed; the date(s) of the hazard assessment; and, which identified the document as a certification of hazard assessment.  a) Facility; on or about January 31, 2018 and times prior, failed to document that a workplace assessment had been performed to determine the hazards, and the necessary personal protective equipment required for the task.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedures and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4):  (a) Facility; on or about January 31, 2018 and times prior, the employer failed to develop and implement an energy control program to protect employees from hazards associated with the release of stored energy when performing service and maintenance activities on machines such as, but not limited to concrete form machines.
