29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed:    (a)  There were 110-VAC insulated wires, entering and exiting the sides of two plastic outlet boxes hanging on the outside fence on the northeast side of the building, that were exposed to abrasions.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.    (a)  The covers on the two plastic outlet boxes hanging on the outside fence by the northeast corner of the building, were not securely attached to the front of the boxes, thereby exposing the 110-VAC insulated wires inside the boxes to damage.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated:    (a)  The extension cords (110-VAC) used outside to warm the truck engines, were not inspected on a daily basis.
