29 CFR 1910.272(m)(4): The employer did not implement procedures in the grain handling facility for the use of tags and locks which would prevent the inadvertent application of energy or motion to equipment being repaired, serviced, or adjusted, which could prevent injuries to employees: The employer is failing to protect employees from amputation hazards by not implementing Lockout/Tagout procedures when working in or around an auger. This was most recently documented at 501 Division Ave, York, Nebraska, prior to January 23, 2018, where employees were not locking out a molasses auger prior to cleaning it.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS) The employer is failing to protect employees from an electrical hazard associated with accessing a 480 volt circuit board without using personal protective equipment. This was most recently identified on January 30, 2018 at 501 Division Ave, York, Nebraska when employees were applying lockout tags to a circuit breaker on a 480 volt circuit breaker panel and they were not using personal protective equipment.
