29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a) On or about January 31, 2018 and at times prior thereto employees were exposed to fall hazards while working from a 7 foot 5 inch high catwalk where a guardrail was not in place around the entire perimeter of the catwalk, nor was any other means of fall protection utilized. b) On or about January 31, 2018 and at times prior thereto employees were exposed to fall hazards while working from corner line pallet stacks, greater than 8 feet high, where no fall protection was used or provided. NOTE-Abatement certification is required for this item.
