On or about January 31, 2018, the walking-working surface floor overlay west of the main entrance adjacent to and in the north-south aisle had delaminated from the base concrete floor causing an uneven surface that exposed employees to slip and trip hazards. 29 CFR 1910.176(a): Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked:  a) On or about January 31, 2018, the permanent aisle adjacent to the Chip/Oils Area is not clearly marked to separate the chip and oil work areas from the aisle used by powered industrial trucks.  This condition exposed employees to struck-by hazards.  b) On or about January 31, 2018, areas of the surface floor overlay west of the main entrance in the north-south aisle had delaminated from the base concrete floor an uneven surface for forklift traffic. This condition exposed employees to struck-by hazards.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
