29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment(s) had been performed through a written certification which included the requirements in paragraph (d)(2) of this section:  Employees engaged in machining activities throughout the Main, South and East building areas were exposed to burns, lacerations, and cuts hazards in that the employer did not verify that the required workplace hazard assessment had been performed through a written certification which included and identified the following:    (1)  The workplace evaluated; (2)  The person certifying that the evaluation had been performed; (3)  The date(s) of the hazard assessment; and, (4)  The document identified as a certification of the hazard assessment.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedures and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4):  Authorized and affected employees in all 3 buildings of the facility were exposed to electrical, struck-by and caught-between hazards associated with the unexpected energizing or start-up of the equipment in that written program for control of potentially hazardous energy on equipment had not been developed.  29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Employees engaged in machining activities at the following pieces of equipment throughout the facility were exposed to struck-by and amputation hazards in that machine guarding was not provided and utilized:  a) Fadal machining area in the Main building, the electrical interlock on the door of the Fadal VMC 3016 CNC machine #696, Model #CNC88HS had been bypassed. b) Bertsche, Okuma, and Mori Seiki machining areas in the Main building, the electrical interlocks on the doors of the Bertsche, Okuma, and Mori Seiki CNC machines had been bypassed. c) Makino and Okuma machining areas in the South building, the electrical interlocks on the doors of the Makino and Okuma CNC machines had been bypassed. d) Toyoda machining area in the East building, the electrical interlocks on the doors of the Toyoda CNC machines had been bypassed. e) Both of the Bridgeport Mills in the Small Part area of the Main building, employees were not provided protection from flying chips and debris, the rotating drill chucks were not guarded. f) Both of the Do-All Band Saws in the Small Part area of the Main building, the unused portions of the blades on the two Band-saws were not guarded.   29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.
