29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about 02/01/18 in the bakery area; employees were exposed to amputation hazard while cleaning the divider when they relied on the interlocked doors and emergency stop button as the method of hazardous energy control.    b) On or about 01/01/18 in the bakery area; employees were exposed to a caught between hazard while cleaning the rounder when they relied on the emergency stop button as the singular method of controlling hazardous energy.    c) On or about 02/01/18 in the bakery area; employees were exposed to an amputation hazard while cleaning the molder when they relied on the interlocked guard as the singular method of controlling hazardous energy.    d) On or about 02/01/18 in the bakery area; employees were exposed to an amputation hazard while cleaning the blades on the slicer when they relied on the emergency stop and interlocked doors as the method of hazardous energy control.   e) On or about 02/01/18 in the bakery area; employees were exposed to an amputation hazard while cleaning the mixer when they relied on the emergency stop and interlocked doors and the method of hazardous energy control. 29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating scraper blade inside the strainer basket:  a) On or about 02/01/18 in the warehouse area; employees were exposed to an amputation hazard while using the raisin washer when the opening at the bottom of the discharge chute was a size and shape which allowed an employee access to the rotating scraper blade inside the strainer basket.   b) On or about 02/01/18 in the bakery area; employees were exposed to an amputation hazard while walking near the back side of the proofer cabinet when the conveyor and moving parts were missing a guard.    c) On or about 03/01/18 in the slicer/bagger line #1; employees were exposed to an amputation hazard when ingoing nip points at the discharge end of the slicer feed conveyor was not guarded.    d) On or about 03/01/18 in the slicer/bagger area; employees were exposed to an amputation hazard when the ingoing nip points on the discharge end of metal detector feed conveyor was not guarded.
