The operator was observed with the load engaging means raised while the rough terrain forklift was unattended.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about January 26, 2018, at a worksite located at 3308 Brecksville Road, Richfield, OH 44286, during commercial construction activities employees were exposed to fall hazards of approximately 10 feet. The employer failed to ensure fall protection systems/devices were used by all employees working on the roof.
