29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:       On or about January 29, 2018, at the job site employees were observed not wearing safety glasses while using hammers and pneumatic nail guns to perform framing works on a single family home.   Employees were exposed eye injury hazards.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:        On or about January 29, 2018, at the job site employees were observed scaling the framing of a single family home in order to access different levels of the building where ladders were not provided.  Employees were exposed to fall hazards.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:         On or about January 29, 2018, at the job site employees were observed not wearing hard hat while working on lower levels in the areas where there was a possibility of head injury caused by falling objects from other employees working above.        Rasar Framing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.100(a), which was contained in OSHA inspection number 1257012, citation number 1, item number 1, and was affirmed as a final order on December 18, 2017 with a final abatement date of November 24, 2017, with respect to a workplace hazard at 1735 E. 31st Street S., Tulsa, OK 74110.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):         On or about January 29, 2018, at the job site employees were working on top of a single family home 6 feet or more above lower levels installing framing materials.  The employees were not wearing personal fall arrest system and had no other type of fall protection system provided.  Employees were exposed to fall hazards.        Rasar Framing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1257012, citation number 1, item number 2c, and was affirmed as a final order on December 18, 2017 with a final abatement date of November 24, 2017, with respect to a workplace hazard at 1735 E. 31st Street S., Tulsa, OK 74110, in OSHA inspection number 1248267, citation number 1, item 1a, and was affirmed as final order on October 26, 2017with a final abatement date of October 13, 2017, with respect to a workplace hazard 14615 E 122nd Place, Owasso, OK 74055, and in OSHA inspection number 1246130, citation 1, item 1a, was affirmed as final order on October 26, 2017 with a final abatement date of October 16, 2017, with respect to a workplace hazard 11314 N 147th East Ave., Owasso, OK 74055.
