29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system:    On or about January 29, 2018, AEP/Public Service Company of Oklahoma building, the employer did not ensure that an employee assigned to clean the atrium area with unprotected sides was protected from falling by guardrail system, safety net system, or personal fall arrest system.  An employee was exposed to the hazard of falling approximately 15 feet to floor below.

29 CFR 1910.30(a)(3)(i): The employer did not ensure that employees were trained in the nature of fall hazards in the work area and how to recognize them:    On or about January 29, 2018, AEP/Public Service Company of Oklahoma building, the employer did not ensure that an employee assigned to clean the atrium area with unprotected sides had been trained in the nature of fall hazards in the work area and how to recognize them.  The employee was exposed to the hazard of falling approximately 15 feet to the floor below.
