29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) Zimkor, LLC: On or about January 31, 2018, and at times prior, employees were exposed to laceration and amputation hazards due to lack of adequate guarding due to working next to exposed blades on the W. F. Wells & Sons aka Wellsaw (Big Saw).        (b) Zimkor, LLC: On or about January 31, 2018, and at times prior, employees were exposed to impact hazards due to open access to moving parts at the rear of the Plate Shear.      (c) Zimkor, LLC: On or about January 31, 2018, and at times prior, employees were exposed to impact and amputation hazards due to lack of point of operation guarding on the Piranha Punch Presses. 29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a)  Zimkor LLC Fabrication Area:  On or about January 31, 2018, and at times prior, employees were exposed to potentially hazardous energy due to lack of specific procedures.  Procedures were not developed for tasks including, but not limited to: changing the blade on the W.F. & Sons Wellsaw horizontal band saw and changing the blade on the Marvel Series 81 vertical saw blade. 29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made:    (a) On or about January 31, 2018, and at times prior, workers were potentially exposed to electrical hazards due to a damaged flexible cord on the spark plug cleaner.  The outer insulation of the flexible cord was damaged in two spots, exposing the inner insulation at both locations.
