Near Bent 11, On or about February 01, 2018, employees were exposed to a struck-by hazard while tying rebar which was suspended from four jack stands via eight mechanical hoists. The working load of the jack stands/rigging equipment was unknown and the identification markings were not affixed to the equipment.
