29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) Glue mix station - On or about 1/26/18, an employee's eyes were exposed to zinc chloride particles, which causes category 1 serious eye damage, when it was mixed with furfuryl alcohol to make an adhesive.   The employee was not been provided with appropriate dust goggles.  ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.1200(g)(8): The employer did not ensure that safety data sheets were readily accessible to the employees in their work area during each work shift:  a) On or about 2/5/18, employees have exposure or potential exposure to hazardous chemicals such as, but not limited to, furfuryl alcohol (a Category 4 flammable liquid) and zinc chloride (a Category 1 serious eye damage).  The computer in the lunch room was not working.  This computer provided employees with access to safety data sheets.  NO ABATEMENT CERTIFICATION REQUIRED.

29 CFR 1910.1200(h)(1): Employees were not provided with effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) Glue mix station - On or about 1/26/18, furfuryl alcohol (a Category 4 flammable liquid) and zinc chloride (a Category 1 serious eye damage) were mixed in a 5 gallon bucket.  The reaction generated heat and gases.  The plant was evacuated.  Employees were not provided with effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that employees had not been previously trained about was introduced into their work area.  NO ABATEMENT CERTIFICATION REQUIRED.
