29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): A.) Meadville Forging Company LP - On or about 1/31/18, and at times prior thereto, the company's lockout/tagout procedures were not specific. The procedures were missing elements including, but not limited to, verifying de-energization, ensuring that affected employees were made aware of the lockout, magnitudes of energy and sequential steps to lockout/tagout the equipment. Equipment with defective procedures included, but was not limited to, 1000 ton Ajax press, 1600 Ton press, 100 ton Erie press, 200 ton Niagara press, Niagara trim press, 500 Hill Acme shear, 1000 Advance Shear and Kaeser compressor.
