29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide, and the employer could not demonstrate that a wider space was necessary: a) Jobsite, 7204 Old McKnight Road, Pittsburgh, PA 15205: On or about January 31, 2018, each platform on all working levels of a scaffold located in front of an exterior southwest wall was not fully planked. An employee working on the top level of the scaffold was exposed to a fall hazard of approximately 35 feet to the ground below.

Employees working on lower levels of the scaffold were exposed to struck-by hazards.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: a) Jobsite, 7204 Old McKnight Road, Pittsburgh, PA 15205: On or about January 31, 2018, guardrail systems were not installed along all open sides and ends of a scaffold located in front of an exterior southwest wall. On all working levels there were some areas of the scaffold that were not equipped with guardrails. This included areas of the scaffold near the access ladders, end frames, and along the sides. An employee working on the top level of the scaffold was exposed to a fall hazard of approximately 35 feet to the ground below.
