29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about December 8, 2017, employees were exposed to fall hazards due to lack of a guardrail system, safety net system, or personal fall arrest system while engaged in residential construction activities more than 6 feet above a lower level. Employees were observed installing roofing materials on a second story roof of a multi-family residence while wearing fall harnesses that were not attached to a lanyard or anchor. 29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. (a) On or about December 8, 2017, employees were exposed to fall hazards due to lack of a guardrail system, safety net system, or personal fall arrest system while engaged in residential construction activities more than 10 feet above a lower level. Employees were observed walking into and standing in the bucket attachment on a forklift to retrieve materials while installing roofing materials at heights greater than 10 feet. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On or about December 8, 2017, and at times prior, employees were exposed to fall from height hazards due to portable ladder side rails not extending at least 3 feet above the upper landing surface. Employees used a portable ladder to access a second story roof. At the time of inspection the ladder side rails extended approximately 12" above the upper landing surface.
