29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): (a) On or about January 29, 2018, and at times prior, the controlling contractor, Villagree Homes and Real Estate, LLC, did not ensure Perry & Sons Construction, LLC and other subcontractors, protected their employees from construction hazards including, but not limited to falling and struck-by, in that their accident prevention program did not include frequent site inspections to prevent persistent and reoccurring hazards.

29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: (a) On or about January 29, 2018, Villagree Homes and Real Estate, LLC, as the controlling contractor on site, allowed the work area to be disorganized and unkempt, exposing employees of Perry & Sons Construction, LLC to punctures from protruding nails and slips, trips and fall hazards.
