29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:    a) On or about February 1, 2018: Three employees were observed operating Hitachi pneumatic nail guns when installing asphalt roof shingles and were not provided with eye protection. Employees were exposed to eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) On or about February 1, 2018: On a 7:12 pitch roof six employees were observed installing asphalt roof shingles and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor was employee provided with an alternative fall protection measure, the employee was exposed to a 11-feet fall hazard.    Roofing Pioneers, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1216740, citation number 1, item number 1 and was affirmed as a final order on June 30, 2017, with respect to a workplace located at 421 N. Woodland Boulevard Deland, FL 32723.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) On or about February 1, 2018: Six employees were observed performing roofing activities at heights higher than 6-feet from next lower level which exposed them to a fall hazard and such employees were not provided with training that will allow them to recognize such hazards and equipped them on how to minimize them.
