29 CFR 1917.42(a)(2): All loose gear, such as slings, chains, bridles, blocks, and hooks, was not inspected by the employer or their authorized representative before each use and during its use, to ensure that it was safe: a) On or about February 6, 2018 on the overhead crane, a four legged wire rope bridle was used to move a 22,000 pound Cat fork truck into a barge. One of the thimbles was deformed, and no inspection tag was present on the bridle.
