29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) 100 Emancipation Dr., Building 48, Hampton, VA 23667: On or about February 1, 2018, each employee that performed work from a ladder jack scaffold approximately 15' above the ground was not protected by a personal fall arrest system, exposing employees to fall hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area: a) 100 Emancipation Dr., Building 48, Hampton, VA 23667: On or about February 1, 2018, and times prior thereto, each subcontractor employee that performed work while on a ladder jack scaffold was not trained in fall hazards and the procedures to be followed to minimize such hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used: a) 100 Emancipation Dr., Building 48, Hampton, VA 23667: On or about February 1, 2018, each employee that accessed a ladder jack scaffold platform approximately 15' above the ground was not provided a safe means of access. Employees accessed the scaffold platform by climbing over said platform from the support ladders. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
