Bagging Department - The chain and sprocket on the Polystar 9500 Bag Machine was not enclosed to prevent contact by employees. Purestat Engineered Technologies, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.219 (f)(1), which was contained in OSHA inspection number 917362, citation number 1, item number 13 and was affirmed as a final order on 08/08/2013, with respect to a workplace located at 21 Old Farm Road, Lewiston Maine 04240.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or loss of an eye, as a result of a work-related incident, within (24) twenty-four hours. 21 Old Farm Road - An employee sustained a fingertip amputation after reaching into a piece of machinery. There was no report of an amputation filed with OSHA within 24 hours.
