Section (5)(a)(1): The employer failed to furnish to each of his employees employment and a place of employment which was free from recognized hazards that were causing or are likely to cause death or serious physical harm, in that employees were exposed to fall hazards : (a) On or about January 31, 2018 for an employee who was injured while being hoisted approximately 20 feet above the rig floor when the swivel of his hoist-line became hung on the tubing board of the servicing rig located at the Olsen 2-8H well site near Keene, North Dakota. Abatement note: Among other feasible and acceptable means of abatement, the employer can implement the following recommendations found in Chapter 19 of the IADC HSE Reference Guide, which advise the employer to: 1. Evaluate the lifting operation to ensure that the path of the line does not come into contact with the rig structure. 2. Ensure operators are inspecting the line and rigging to evaluate the potential for components to snag during the lift. 3. Use a tagline to reduce swaying and the potential for entanglement during the lift. 4. Consider weather conditions, to include the effect of wind on the operation, when planning and conducting lifts.
