29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:     a) Churchmen's Machine Company, Inc. - On or about February 6, 2018, the employer failed to develop specific energy control procedures to be followed when service or maintenance, including set up and cleaning, is performed on energized equipment or machinery.     Abatement Note:  The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  - A specific statement of the intended use of the procedure;   - Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;   - Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and   - Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.     The procedure must contain information which authorized employees must know to safely control hazardous energy. Over generalization can result in a document which has little or no utility to the employee who must follow procedure. However, while the procedure is required to be written in detail, this does not mean that a separate procedure must be written for each and every machine or piece of equipment. Similar machines and/or equipment (those using the same type and magnitude energy) that has the same or similar types of controls can be covered with a single procedure.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:     a) Chrurchmen's Machine Company, Inc.- On or about February 6, 2018, the employer failed to ensure an inspection of the written energy control procedure was conducted at least annually.     Abatement Note:      1.  Where lockout is used for energy control, the periodic inspection shall include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected.1910.147(c)(6)(i)(D)     2.  Where tagout is used for energy control, the periodic inspection shall include a review, between the inspector and each authorized and affected employee, of that employee's responsibilities under the energy control procedure being inspected, and the elements set forth in paragraph (c)(7)(ii) of this section.1910.147(c)(6)(ii)     3.  The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    a) Churchman Machine Company, Inc. - On or about February 6, 2018, the employer failed to provide information and training related to the hazards of chemicals used in the workplace.  Employees are exposed to hazardous chemicals to include, but not limited to, cutting fluids, lubricants, and welding rods.    b) Churchman's Machine Company, Inc. - On or about February 6, 2016,the employer did not provide employees who handle hazardous chemicals training, including and explanation of shipping labels and the workplace labeling system, nor the new safety data sheet format which was instituted in December of 2013.
