29 CFR 1910.145(c)(3):     Safety instruction sign(s) were not used where there was a need for general instructions and suggestions relative to safety measure(s):    On or bout February 6, 2018, in the lot adjacent to the terminal located at Pitt Ohio, LLC in Harrisburg, PA, safety instruction signs were not in place at the crosswalk to the terminal office.      ------------------------------------------------------------------------------------------------Originally issued as     OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from struck by hazards:     a) On or about February 6, 2018, in the lot adjacent to the terminal, located at Pitt Ohio, LLC in Harrisburg, PA, a Pitt Ohio truck driver, wearing dark clothing at night exited his vehicle to return to the terminal office when he was struck by another Pitt Ohio truck driver. The employer did not ensure that employees wore high-visibility apparel while walking or working during hours of darkness in all areas of customer yards and terminal lots. The employee was exposed to a struck-by hazards from a moving vehicle.
