29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a)  Employees were exposed to entanglement hazards from rotating parts while working on and around the Ikegai TV4 Milling Machine (Model No. TV4 HR and Serial No. 53524V), and the employer did not ensure the point of operation was properly guarded.       (b)  Employees were exposed to entanglement hazards from rotating parts while working on and around the Ikegai TV5 Milling Machine (Model No. TV5 R and Serial No. 53515V), and the employer did not ensure the point of operation was properly guarded.       (c)  Employees were exposed to entanglement hazards from rotating parts while working on and around the Ikegai TV4 Milling Machine (Model No. TV4 SR and Serial No. 53443V), and the employer did not ensure the point of operation was properly guarded.      (d)  On or about February 7, 2018, the safety interlocks were bypassed on the Bridgeport VMC CNC machine (Model No. VMC 1000 22 and Serial No. 822723) which exposed employees to an entanglement hazard from rotating parts.      (e)  On or about February 7, 2018, the safety interlocks were bypassed on the Milltronics TT 24 CNC machine (Model No. TT24 and Serial No. 10768) which exposed employees to an entanglement hazard from rotating parts.      (f)  On or about February 7, 2018, the safety interlocks were bypassed on the Mytrunnion CNC machine (Model No. Mytrunnion and Serial No. 10-027) which exposed employees to an entanglement hazard from rotating parts.      (g)  Employees were exposed to entanglement hazards from rotating parts while working on and around the Leadwell V-40 iT CNC machine(Model No. V-40 iT and Serial No. L2TAB0136), and the employer did not ensure the point of operation was properly guarded.       (h)  Employees were exposed to entanglement hazards from rotating parts while working on and around the Leadwell V-40 iT CNC machine(Model No. V-40 iT and Serial No. L2TAC0495), and the employer did not ensure the point of operation was properly guarded.       (i)  Employees were exposed to entanglement hazards from rotating parts while working on and around the Leadwell V-20 iT CNC machine(Model No. V-20 iT and Serial No. L2TAB0258), and the employer did not ensure the point of operation was properly guarded.       (j)  Employees were exposed to entanglement hazards from rotating parts while working on and around the Leadwell V-20 iT CNC machine(Model No. V-20 iT and Serial No. L2TAJ1207), and the employer did not ensure the point of operation was properly guarded.       (k)  Employees were exposed to entanglement hazards from rotating parts while working on and around the Leadwell V-30 CNC machine(Model No. V-30 and Serial No. L2TJE0771), and the employer did not ensure the point of operation was properly guarded.
