This was most recently documented on or about February 1, 2018 at a jobsite located at or near Highway N and Welsh Drive, Lake Saint Louis, MO 63367. The competent person: (a) Allowed employees to enter an excavation without a protective system in the excavation that measured approximately 9 feet in depth, 15 feet in width and 18 feet in length. (b) Authorized employees to enter excavations without providing access and egress path.
